If you are interested in holding a seat for your child in any of our programs, please print off the proper registration form and return it with the appropriate fees. The rest of the documents will be required before the first day of school.

PRESCHOOL REGISTRATION PACKET
The following items are needed to fully enroll your child:

• Registration Form
• Registration Fee- $70/child or $100/family
• Emergency Information & Immunization Form
• Parental Agreement Form
• Playground Liability Form
• Copy of Immunization Record
• Individual Photo of child (no larger than 4x6)
• Custody Papers (if applicable)

BEFORE/AFTER CARE REGISTRATION PACKET
The following items are needed to fully enroll your child:

• Preschool Before & After Care Registration Form
• EAGLE Harmony Registration Form
• EAGLE Prep Registration Form
• Registration Fee- $10/child
• Emergency Information & Immunization Form
• Playground Liability Form
• Copy of Immunization Record
• Custody Papers (if applicable)
